Local anaesthetic systemic toxicity following oral ingestion in a child: Revisiting dibucaine
نویسندگان
چکیده
Dibucaine, a potent and toxic local anaesthetic, although currently withdrawn by the United States Food and Drug Administration for use as a spinal anaesthetic, continues to remain available in many over-the-counter topical formulations. Systemic toxicity following oral ingestion of local anaesthetics is rare. We report a case of accidental ingestion of dibucaine (ear drops) in a 7-year-old child who developed diplopia, giddiness, ventricular premature contractions and a right bundle branch block. We also present a brief discussion on the pharmacologic and toxicity profile of dibucaine, the Naranjo algorithm for assessing causality in case of adverse drug reactions and a review of current guidelines on the management of local anaesthetic systemic toxicity.

Chlorhexidine is bonded well to the oral mucosa and dental pellicle and is poorly absorbed from the astrointestinal tract, but in high concentration it is absorbed enough to produce liver necrosis. In this case a dentistry student accidentally ingested a shot of 20% chlorhexidine gluconate solution. Treatments included washing the oral cavity with lots of tooth paste, drinking of 5% alginate sy...
متن کاملPHARMACOLOGICAL STUDIES ON NOVEL BASIC AMINOALKYL ARYL ETHERS AS POTENTIAL LOCAL ANAESTHETICS
Following our previous investigations on certain new aminoalkyl aryl ethers, which showed promising and outstanding local anaesthetic properties, the local anaesthetic activity and duration of action of a further selection of four novel derivatives of the above-mentioned series, used as hydrochloride salts, have been determined by the ill vivo rat sciatic nerve test. These are: N,N-dimethyl...

متن کاملLipid Emulsion for Local Anesthetic Systemic Toxicity
The accidental overdose of local anesthetics may prove fatal. The commonly used amide local anesthetics have varying adverse effects on the myocardium, and beyond a certain dose all are capable of causing death. Local anesthetics are the most frequently used drugs amongst anesthetists and although uncommon, local anaesthetic systemic toxicity accounts for a high proportion of mortality, with lo...
